Depression and Thyroid Hormones

By Gabe Mirkin, M.D. If you are tired or depressed much of the time, your doctor should order blood tests for the two thyroid hormones called T3 and T4 and for the brain hormones called TSH and prolactin. If your TSH is high and your prolactin is normal, you are probably hypothyroid and need to take thyroid hormone to give you more energy and prevent heart and blood vessel damage. Doctors treat people with low thyroid function with thyroid pills called T4 (Levothroid, one brand name is Synthroid). Many doctors think that a person needs only T4 because the thyroid gland makes T4 and then it is converted to T3 in other tissues. However, some people become depressed when they take just T4 and their depression can be cured when they take both thyroid hormones, T3 and T4. When a depressed patient comes to me and is taking thyroid hormone, T4, I immediately order a blood test called TSH to check if he or she is getting the correct dose. If the TSH is normal, I reduce the dose of T4 by 50% and add a very low dose of T3 (brand name, Cytomel) because it safer to prescribe too low a dose, rather than too high a dose. Overdoses cause shakiness, irritability, irregular heart beats, clots, and osteoporosis. The patient returns in one month for a blood test, TSH, to see if the total thyroid dose is correct. If the TSH is too high, the thyroid dose is too low and I raise the T3 (Cytomel) dose by 5 to 10 m5 each month until the TSH is normal. Then once a year I check TSH blood levels to make sure that the person’s requirements for thyroid hormone are being met. For example, the usual replacement dose for low thyroid function is 100 micrograms per day. If a depressed patient has a normal TSH, I reduce the T4 dose to 50 mcg/day and add 5 mcg of T3 per day. One month later, if the TSH blood is still too high I raise the T3 dose to 10 or 20 mcg and continue to increase the T3 level each month until the TSH is normal. Exciting research shows that the thyroid hormone called T3 can help treat depression. Psychotherapy often fails to control depression. Sigmund Freud, the father of psychotherapy, proposed theories about depression, that many psychiatrists do not accept because his writings were his opinions and not presented as scientific data supported by controlled experiments. The dominant theory today is that depression is caused by low brain levels of the neurotransmitters, serotonin and norepinephrine. The drugs such as Paxil, Prozac and Zoloft that treat depression are supposed to raise brain levels of these neurotransmitters. Doctors can also raise brain levels of serotonin by prescribing pills containing T3, a hormone produced by peripheral tissue from T4, which is produced by the thyroid gland. They also prescribe T3 by itself or together with antidepressants. Depression is common among people who have too much or too little thyroid hormone. Doctors usually treat low thyroid function with T4 also known as Levothroid and many people become even more depressed. They treat this depression by prescribing T3 as well as T4. Try to balance T3 and T4 so you will not be taking too much thyroid and harm yourself. 1)If you now take 100 mcg of Levothroid (T4): 2) Lower T4 (Levothroid) to 50 mcg and add Cytomel (T3) 5 mcg each day. 3) One month later, have your doctor draw blood for TSH. 4) If it is normal, you are on the correct dose and should get blood tests TSH once a year. 5) If TSH is too high, increase Cytomel to 10 mcg and hold Levothroid at 50. 6) Draw monthly TSH until it is normal. Keep on raising Cytomel by 5 mcg until TSH is normal. It is said that standing on your feet too long or even heredity is a good explanation for the veins. Spider veins will become more of a problem with age and are found more often in women than in men. To treat spider veins, you can try to wear support hose if you are going to be doing a lot of standing. This is a great idea if you are someone that stands on your feet as part of your job. The support hose will give your legs compression that they need so that you do not form more of these veins or have them cause you pain. Exercise is a great way to help with the spider veins. Make sure that you are stretching your legs and getting plenty of movement so that you are keeping the blood flowing through your legs. You should also make sure that you are wearing comfortable shoes when you have to be on your feet for longer periods of time. There is a treatment called Sclerotherapy. This is the best and safest treatment for spider veins. It will cause little pain takes little time. A concentrated salt or chemical solution is injected with a small needle into the vein. This causes the vein to pinch and close up. It will later collapse and become part of scar tissue that is taken in by the body. You will have to have more of the treatments after that, but it will eventually work. There are also lasers that can now treat spider veins. You will see that there are options of there to help in this problem and there is no reason to live with them and be self conscious about your legs anymore. And while that is partially true, it does not make up the majority. Approximately 80% of the United States is made up of small to medium size enterprises or businesses called SMEs. Many of these SMEs can not afford the cost of providing insurance coverage to their employees any more than the employees can cover the cost of premiums on their own. Many of these uninsured and underinsured are the working population, the retired, the elderly, those on fixed incomes, single parents, and yes, the working poor. But these folks are not the only people who can not afford medical and dental insurance. The owners of the SMEs can not afford insurance for their own families as they struggle to compete in todays increasingly competitive global market. Even people who have jobs that help cover some of the cost of insurance premiums find they are being asked to assume more and more of the cost yearly. Even big business is struggling under the cost of health insurance in the United States. Increasing medical costs and decreasing health benefits, is taking a heavy toll on those with either no insurance or a limited coverage. At an age when health benefits are most sought, people struggle to stay healthy instead, for fear of rising medical bills. They are offered the very best care as they know their reputation depends on what is brought back to the U.S. So many people are taking vacations and getting treatment in the medical tourism that its difficult to move fast enough. It is very important that the client talk with the doctor about their procedure, that the doctor reviews the files of the patient and that the patient checks out the hospital and physician thoroughly. Businesses have seen a huge potential with their service. Many of the physicians in these foreign hospitals have received the very same training as U.S. physicians, in the very same universities, and then went home to practice their craft. Countries like India which are actively promoting medical tourism are increasingly seeking JCI certification, to instill faith in the medical tourists coming to them, very well realizing that the publicity generated by one surgery could de-rail a burgeoning industry, expected to jump 30 percent every year. Besides the hospitals equipped with the latest medically advanced diagnostic equipments, Indian pharmaceutical companies also meet stringent requirements of the US Food and Drug administration. Medical advancements have meant that Indian doctors can now perform the hip re-surfacing surgery among others where the damaged bone is scraped away and replaced with chrome alloy, an operation which costs less and causes less post-operative trauma than the traditional hip replacement procedure done in the US. By Denice Moffat How do you know when youve picked the perfect gift? Do you wait for a smile? A romantic inflection of the voice? A warm hug, and a big thank you? Isnt that the ultimate goal? To please the people you love the most? What if there were a way to always pick the perfect gift? Well, there is One technique that works well at our house is the To Buy File. We each have one. In the file we put in pictures of what we want to have, write down titles of books, CDs, places we want to go or workshops wed like to experience. We put in pictures of things we only dream wed have some day as well. Included is a wide-range of items with an equally wide range of prices. Sometimes we even include honey do stuff, like things wed like to see get done around the house, a massage, or a romantic dinner at a local restaurant. Now, when a special occasion arises (like Christmas, birthdays, or other holidays), I just go to my husbands To Buy file and pick out a few things that are within my predetermined budget. To Buy Files need to be in a place where everyone can access them. If you are anything like me, I find that I sometimes spend more money for other people than I should, even though I purchase a Christmas present or two each month throughout the year. So when someone asks me what I want, I used to automatically say, Oh, I dont need anything, or Id tell them I wanted something cheap (which I really didnt want) because I didnt feel like there was any money to spend. Now I say, Look in my, To Buy file. Last fall, I took the concept a step further. I organized these items in list form on my computer putting the items I would prefer most to have most at the top of the list. Giving is good, but spiritual development only progresses when the process goes both ways. The To Buy File is one way of asking for and getting what you really want. Let the person who is buying for you decide how much they can afford. They love you and want to please you. Dont take that pleasure of giving away from them. Let them pamper you.
